This is where the external conflict is introduced. After sneaking out, she meets Rotti, who promises to cure her blood disease. Shilo has a blood disease, passed down from her mother. It is then revealed to us the true identity of the Repo Man, a man named Nathan Wallace with a daughter named Shilo. But, it seems there is a history between this Repo Man and Rotti. We then cut to a man in a mask who, unfortunately, takes back the organs that the customers had if they aren't able to pay Geneco back.
He then erects Geneco, a company where you can get your organs replaced. A man by the name of Rotti Largo, depicted as the man in the suit, strikes a deal with the government to make the buying and selling of organs.
We are introduced to the setting - a world ravaged by a disease that makes your organs fail.
